High School Sports

Physicals are required in order to try out for any athletic team this year, and strength/conditioning sessions or open gyms in the summer. Students who plan on playing any sport, even a winter or spring sport, should get their physical completed as soon as possible. You can pick up/fill out all paperwork in the front office or print it from the links below. The following forms are required for the physical:

LCMC Sports Medicine Physical Information: 

The athletic fee per sport per participant is $125 and can be paid online at the Sports Fees link. Students who participate in multiple sports will only be billed $250.
